As a Case Manager – Crisis Housing, you’ll provide short-term accommodation solutions and case management support to those experiencing or at risk of homelessness. Working within a proactive outreach model, you’ll assist clients in transitioning to safe, secure, and sustainable housing-whether through social housing, private rental, or homeownership.
You’ll be part of the Homelessness Services, Rural and Remote team, working closely with other professionals to provide high-quality support tailored to each client’s needs.
About the Service
The Homelessness Services team in Mount Isa delivers immediate and long-term housing support, using a client-centered approach to empower individuals. This service operates within a responsive and collaborative framework, ensuring clients receive the advocacy, referrals, and resources needed to achieve housing stability and overall wellbeing.
Key Responsibilities
- Provide comprehensive case management, including assessment, planning, and goal setting
- Develop early intervention strategies to support clients in achieving housing stability
- Conduct eligibility assessments and ongoing progress reviews using the Outcome Star tool
- Facilitate referrals to housing providers, community organizations, and specialist services
- Advocate on behalf of clients to secure stable accommodation and essential services
- Build and maintain strong community partnerships through networking and stakeholder engagement
- Maintain accurate records in line with service agreements, legislation, and compliance requirements
- Provide outreach and crisis support, ensuring immediate housing needs are met
- Contribute to program development, community education, and service improvement initiatives
- Attend team meetings, professional development, and supervision sessions
- Ensure compliance with workplace health and safety, privacy, and equal opportunity policies
Skills & Requirements
- Degree in Social Work, Human Services, or Behavioural Sciences OR a Certificate IV or higher in Community Services with 2+ years of case management experience
- Strong communication, advocacy, and problem-solving skills
- Knowledge of housing pathways and community resources
- Experience working with culturally diverse communities
- Valid Open Driver’s License
- Current Working with Children Blue Card & Disability Services Yellow Card (or ability to obtain)
Desirable:
- Experience in group facilitation or multidisciplinary teams
- Flexibility to work outside standard office hours if required
- Previous experience in homelessness, social services, or NFP organizations
