About the Role:
As Clinical Care Coordinator you will be primarily responsible for the clinical care of care recipients within the facility. Working within the leadership team, supported by an experienced and passionate residential & care manager within this medium sized care home within the Bundaberg region.
You will provide clinical leadership to nursing and care staff, mentoring and inspiring staff to provide outstanding care to all residents whilst supporting with their ongoing development and career progression
You will utilise your previous experience to oversee the clinical governance of the facility and optimise AN-ACC funding. You will coordinate your team and ensure safe, clear and effective communication across the multidisciplinary team.
Main Duties
- Provide clinical guidance to residents & care staff
- Undertake resident assessments & case conferencing in line with standards
- Accurate completion, review & updating of relevant documentation
- Motivation, training & development of clinical team members
- Ensure compliance & quality indicators are met

Skills & Requirements:
- AHPRA Registered Nurse registration with previous leadership experience
- Staff mentorship & development experience.
- Up to date working knowledge & experience achieving accreditation standards
- Ability to identify & improve areas of clinical risk & quality guidelines
- Consultative & effective communication skills.
