About the Role:

As a Community Clinical Manager, you’ll inspire, mentor, and support your team to deliver exceptional, person-centred care across your region. You’ll be central to building a connected, positive, and compassionate community where every client feels supported and valued.

Using your strong communication and leadership skills, you’ll partner with clients to understand their clinical needs, conduct in-home assessments, and develop care plans that align with individual lifestyles, care packages, and organisational standards – always with a strong focus on quality and compliance.

Drawing on your experience in developing clinical teams, you’ll foster a collaborative culture, identify opportunities for growth, and empower your staff to thrive, learn, and continuously improve.

Main Duties:

  • Provide clinical guidance to participants & care staff
  • Undertake assessments & case conferencing in line with standards & funding streams
  • Accurate completion, review & updating of relevant documentation
  • Motivation, training & development of clinical team members
  • Ensure compliance & quality indicators are met

Skills & Requirements:

  • AHPRA registration as a Registered Nurse with previous leadership experience either within residential or community aged care.
  • Staff mentorship & development experience.
  • Up to date working knowledge & experience achieving accreditation standards
  • Strong understanding of HCP, NDIS & DVA funding streams
  • Consultative & effective communication skills.
