About the Role:
We are seeking a passionate and experienced Nurse Practitioner to support across multiple aged care services to within the Newcastle region of NSW.
This is an opportunity to make a meaningful difference by delivering high-quality, person-centred care to older Australians.
In this autonomous and rewarding role, you will provide advanced clinical assessment, diagnosis, and management of resident’s health needs while working collaboratively with multidisciplinary teams. You will play a key role in improving access to timely care and supporting continuity of treatment across facilities.
Your scope of practice will include prescribing medications, ordering and interpreting diagnostic investigations, and directly referring residents to medical specialists and allied health professionals. Through your clinical expertise and leadership, you will help drive best-practice care and positive health outcomes for residents.
Main Duties:
- Work collaboratively within an multidisciplinary team including GPs, geriatricians and RNs
- Lead advanced clinical assessments, diagnosis and management of complex health needs in older adults
- Provide timely intervention for acute deterioration such as infections, falls and delirium
- Order and interpret investigations, prescribe and de-prescribe medications, and optimise treatment plans
- Deliver proactive chronic disease management, preventive care and coordinated support using digital tools and telehealth

Skills & Requirements:
- AHPRA-registered Nurse Practitioner (endorsed), RN, no restrictions; Medicare Provider Number (or eligible)
- Experience in aged care, geriatrics, community, sub-acute or palliative care
- Confident managing acute and chronic conditions in older adults
- Strong prescribing, clinical judgement and communication skills
- Competent with EMRs and telehealth; NSW driver’s license and able to travel to aged care facilities
